66歳，男性．倦怠感と全身浮腫，両側滲出性胸水を認めたが，血液検査や画像検査，胸水検査で特異的所見は認めなかった．局所麻酔下胸腔鏡検査の結果，胸膜に多発結節性病変があり，病理組織像で非乾酪性類上皮細胞肉芽腫を認めた．気管支鏡検査所見もサルコイドーシスに矛盾なく，サルコイドーシス（組織診断群）と診断した．滲出性胸水を伴う症例で一般的な検査で特異的所見がない場合，局所麻酔下胸腔鏡検査を検討することが重要である．
