66歳,男性.倦怠感と全身浮腫,両側滲出性胸水を認めたが,血液検査や画像検査,胸水検査で特異的所見は認めなかった.局所麻酔下胸腔鏡検査の結果,胸膜に多発結節性病変があり,病理組織像で非乾酪性類上皮細胞肉芽腫を認めた.気管支鏡検査所見もサルコイドーシスに矛盾なく,サルコイドーシス(組織診断群)と診断した.滲出性胸水を伴う症例で一般的な検査で特異的所見がない場合,局所麻酔下胸腔鏡検査を検討することが重要である.
