症例は58歳男性.突然の胸痛の後に,右頸部の疼痛,腫脹が増悪し搬送された.Stanford A型大動脈解離と診断し,腕頭動脈周囲に血腫を認めたことから同動脈の破裂を疑った.右総頸動脈は解離によって真腔閉鎖していた.緊急部分弓部置換術の方針となり,右腋窩,左大腿の2本送血で体外循環を確立したが,術中腕頭動脈が再破裂したため,右総頸動脈に脳灌流用の人工血管を吻合し,それを用いて分枝再建も行った.術後経過は良好で,脳神経合併症を認めることなく退院に至った.
