症例は58歳男性．突然の胸痛の後に，右頸部の疼痛，腫脹が増悪し搬送された．Stanford A型大動脈解離と診断し，腕頭動脈周囲に血腫を認めたことから同動脈の破裂を疑った．右総頸動脈は解離によって真腔閉鎖していた．緊急部分弓部置換術の方針となり，右腋窩，左大腿の2本送血で体外循環を確立したが，術中腕頭動脈が再破裂したため，右総頸動脈に脳灌流用の人工血管を吻合し，それを用いて分枝再建も行った．術後経過は良好で，脳神経合併症を認めることなく退院に至った．
