症例は67歳女性の維持透析患者で心房細動, 心臓弁膜症の既往がある. 左上下肢の脱力にて当院に緊急搬送された. 来院時, 左片麻痺と半側空間無視を認め, 頭部MRI・MRA検査にて右中大脳動脈領域の急性期心原性脳塞栓症と診断した. 発症4時間半後に遺伝子組み換え型組織プラスミノゲンアクチベーター (rt-PA) を静脈投与したが改善なく, 引き続いて血管内治療を行った. 発症7時間後に機械的再開通に成功し, 明らかな後遺症なく第17病日に自宅退院した. rt-PA治療が無効…
